How do you know it’s ready to move into aged care?

The decision to transition into aged care can be overwhelming and emotional. Whether there has been a sudden change in circumstances or you are planning for the future the decision should revolved around ONE aspect ONLY and that is to ensure you or your loved on will live the best lifestyle they possibly can.

What should I consider when deciding to transition into aged care?

You should consider whether or not you or your loved one has the ability to cope with their everyday activities. If not, you should assess what kind of help they would need and where they would get it.

If I am able to cope independently can I still access services?

Yes, if you are able to stay at home you can still receive aged care assistance such as home maintenance, nursing, meals, social and financial services, transport and several others.

You can get more information here www.commcarelink.health.gov.au.
